Document acceptable as Proof of Identity

Form 49A & CSF:-

1. AADHAR Card issued by the Unique Identification Authority of India
2. Arm's License
3. Bank Certificate in original on letter head from the branch containing duly attested photograph and bank account number of the applicant
4. Central Government Health Scheme Card
5. Certificate of Identity in original signed by a Gazetted Officer
6. Certificate of Identity in original signed by Member of Legislative Assembly
7. Certificate of Identity in original signed by Member of Parliament
8. Certificate of Identity in original signed by Municipal Councilor
9. Driving License
10. Elector's Photo Identity Card
11. Ex-servicemen Contributory Heath Scheme photo card
12. Passport
13. Pensioner Card having photograph of the applicant
14. Photo Identity Card issued by the Central Government or a State Government or a Public Sector Undertaking
15. Ration Card having photograph of the applicant

Form 49AA:-

1. Passport
2. Person of Indian Origin card issued by Government of India
3. Overseas Citizen of India card issued by Government of India

Note:-

1. Documents must be self attested by the applicant.
2. Documents should be in the full name of the applicant.
3. In case of a person being a minor, any of the above mentioned documents of the parents or guardian of such minor shall be deemed to be proof of identity.
