Document acceptable as Proof of Date of Birth

Form 49A & CSF:-

1. AADHAR Card issued by the Unique Identification Authority of India
2. Affidavit sworn before a Magistrate stating the date of birth
3. Birth Certificate
4. Central Government Health Scheme Card or Ex-servicemen Contributory Heath Scheme photo card
5. Domicile Certificate issued by the Government
6. Driving License
7. Elector's Photo Identity Card
8. Marriage Certificate issued by Registrar of Marriages
9. Matriculation Certificate or mark sheet of recognized board
10. Passport
11. Pension Payment Order
12. Photo Identity Card issued by the Central Government or a State Government or a Public Sector Undertaking

Form 49AA:-

1. Passport
